The FQHC Telehealth Consortium is actively addressing racial, healthcare, and digital inequities by building a robust and sustainable telehealth infrastructure capable of reaching and improving the health of the Commonwealth’s most under-resourced patients.
PHASE 2 FUNDING GOALS
Funding the Future of Healthcare
The Consortium seeks to raise $12M toward its Phase II goals. Below are four major funding areas that can be customized to achieve shared goals with funders.
Telehealth Transformation Initiative
A program to fully integrate sustainable telehealth into primary and behavioral health care at all Consortium member sites. The initiative funds a project manager and provider champion at individual local FQHCs.
Telehealth Navigator Initiative
A community-facing initiative to recruit and train Telehealth Navigators that support patients in engaging with telehealth. The program will build a skilled workforce from the communities it serves.
Connected Care Initiative
The Connected Care Initiative leverages a $3M FCC grant for technology, connectivity, training, patient engagement, and evaluation. The program supports behavioral health and substance use treatment via telehealth through expanded access to broadband in our communities.
Measurement, Evaluation, and Dissemination
We will utilize analytics, including data segmented by race and ethnicity, to measure telehealth use across Consortium members. We will also conduct independent research and evaluation of the impact of telehealth initiatives on access, quality, and health equity. The Consortium will also produce tools to share our knowledge and insights, most notably the FQHC Telehealth Playbook.

PHASE 1 FUNDING ACCOMPLISHMENTS
The Consortium raised $5M during Phase I to support its mission of bridging the digital and health equity divide.
We helped 35 FQHCs provide remote primary and behavioral health care to thousands of patients during the COVID pandemic.
We distributed 1,500 cell phones
and other remote patient
monitoring devices
Our work helped member health
centers capture 65% of their
pre-COVID revenue.
