PHASE 1
In March 2020, Community Care Cooperative and the Massachusetts League of Community Health Centers identified pivoting to telehealth services as a key strategy for meeting the needs of their low-income, diverse communities and for keeping their member health centers financially sound.
WHAT WE’VE ACHIEVED
Our work during Phase I demonstrated the need for FQHCs to integrate telehealth as a long-term capability beyond the COVID-19 emergency.
In November 2020, the Consortium successfully completed Phase I. Here’s what we achieved during the inaugural phase.
Since March 2020, the Consortium has achieved the following:
- We provided the hardware, software and IT support needed for member health centers to maintain their operations via telehealth during the COVID-19 pandemic.
- We provided patient monitoring devices to facilitate remote COVID care for members to monitor patients and determine when hospital care was required.
- We piloted and extended the provision of smart phones, connectivity, and connected devices to 1,500 patients across Massachusetts
- We created a collaborative learning community to share knowledge across member health centers.
- We partnered with our members to conduct assessments of their telehealth capabilities and developed customized plans to advance telehealth maturity for each member.
- We assessed the patient experience with telehealth via a patient satisfaction survey.

A lead grant from the Patrick J. McGovern Foundation helped launch Phase I of the FQHC Telehealth Consortium, which resulted in a successful $5 million campaign to fund robust telehealth services at 35 FQHCs.
CONSORTIUM FUNDERS

- Federal Communications Commission administered COVID-19 Telehealth Program funded by the CARES Act
